Dengue fever can become severe after several days of fever, especially around the time the patient begins to defervesce. Therefore, preventing dengue at home involves more than just spraying insecticide; it also includes eliminating mosquito breeding sites, preventing mosquito bites, and monitoring the patient properly. Below is a practical checklist you can use at home, along with signs indicating when to contact a healthcare facility.

1. Checklist for preventing dengue at home and eliminating mosquito larvae

Dengue-carrying mosquitoes commonly breed in containers holding stagnant water around the home. Even a small amount of water in a bottle cap, plant saucer, or discarded item can become a place for mosquitoes to lay eggs. I recommend checking the entire house and surrounding area at least once a week, especially after rain.

Check and manage water-holding containers

  • Cover tightly tanks, barrels, jars, large pots, and water containers that are not used regularly.
  • Replace the water and scrub the inside of containers regularly; do not simply empty the water, as mosquito eggs can cling to the sides.
  • Turn upside down or discard old tires, cans, bottles, boxes, coconut shells, and other items that can collect rainwater.
  • Clean water-collection trays under plant pots, refrigerators, or air conditioners; if they are unnecessary, remove them or keep them dry.
  • Clear gutters and drainage systems and other areas around the home where water may collect.

This is the core of mosquito-larva control. If you cannot empty or cover a water container, ask local health officials about suitable treatment measures. Do not use chemicals, oil, or larvicidal products without clear instructions in containers used for household water.

Reduce adult mosquitoes indoors

  • Install screens on windows and doors where appropriate, and repair any tears.
  • Use mosquito nets while sleeping, including during the day, especially for young children, sick people, and those who nap.
  • Keep the home well ventilated and reduce damp, dark areas where mosquitoes may hide.
  • Use insecticide sprays, electric mosquito swatters, or mosquito-repellent devices according to the label instructions; move young children and sensitive individuals away from the area when necessary.

2. Preventing mosquito bites when someone in the family has a fever

A person with dengue can carry the virus, which mosquitoes may acquire by feeding on their blood and then transmit to others. Therefore, during the illness, preventing mosquito bites for the patient also helps protect the entire family and community.

  • Have the patient sleep under a mosquito net, including when resting during the day.
  • Wear loose, long clothing that provides adequate coverage.
  • A mosquito repellent may be applied to the skin as directed; carefully read the age restrictions, avoid applying it to the eyes, mouth, or broken skin, and wash your hands after use.
  • Do not let children use mosquito repellent or insecticide sprays themselves unless instructed by an adult.
  • Continue treating water-holding containers around the home, as spraying indoors does not replace eliminating mosquito breeding sites.

If someone in the household has a fever, record when the fever began, the temperature, fluid intake, urination frequency, medications taken, and any newly appearing symptoms. This information helps the doctor assess the progression more accurately.

3. Signs of severe dengue and when hospitalization is needed

Disease severity cannot be determined solely by whether the fever is high or low. Some people may worsen as their temperature falls, usually between days 3 and 7 after symptom onset, but the exact timing varies. If dengue is suspected, take the patient for medical evaluation and any indicated testing rather than relying entirely on monitoring at home.

Warning signs requiring immediate medical attention

The following symptoms should be considered warning signs, especially when they appear just as the patient’s fever is subsiding:

  • Severe or worsening abdominal pain, or frequent vomiting.
  • Lethargy, restlessness, agitation, unusual fatigue, or difficulty waking.
  • Nosebleeds, bleeding gums, vomiting blood, black stools, or unusual bleeding.
  • Rapid breathing, difficulty breathing, chest tightness, or cold hands and feet.
  • Unable to drink, persistent vomiting, very dry lips, or a marked decrease in urination.
  • Pale, cold, mottled skin; dizziness or fainting.

Young children, older adults, pregnant women, and people with underlying conditions such as heart, kidney, or liver disease, diabetes, or those taking medications that affect blood clotting should seek medical advice earlier when they develop a fever.

When is hospitalization or an emergency call needed?

When hospitalization is needed depends on warning signs, examination and test results, the person’s ability to drink and be monitored at home, and their underlying conditions. Do not wait until the patient has severe bleeding before taking them to the hospital. Go to a healthcare facility immediately if any of the warning signs above occur. If the patient has difficulty breathing, is confused or unconscious, faints, has cold extremities, is bleeding heavily, or cannot be transported safely, call your local emergency services.

Care while waiting for medical evaluation

  • Have the patient rest and take small sips of fluid; an appropriate oral rehydration solution may be used as directed.
  • Use the correct dose of paracetamol as advised by a healthcare professional if fever reduction is needed.
  • Do not take aspirin, ibuprofen, or nonsteroidal anti-inflammatory drugs because they may increase the risk of bleeding.
  • Do not administer intravenous fluids at home or take antibiotics or so-called “platelet-boosting” medicines without medical advice.
  • Do not apply cold compresses, wipe the body with alcohol, or use potentially harmful folk remedies.

There is no routinely used specific antiviral medication for dengue; treatment mainly consists of monitoring and supportive care as prescribed. Early test results also do not always rule out the disease, so the doctor may schedule repeat testing based on the day of illness and the symptoms.

A 10-minute weekly checklist for the whole family

  1. Walk around the house and yard and look for all water-holding items.
  2. Empty water from bottles, cans, tires, and discarded items.
  3. Scrub tanks, jars, trays, and water containers that need to be kept.
  4. Keep water containers tightly covered.
  5. Check plant pots, gutters, drains, and the area behind the house.
  6. Turn unused items upside down.
  7. Check screens, door and window nets, and any places where mosquitoes could enter the house.
  8. Prepare mosquito repellents suitable for each household member.
  9. If someone has a fever, record the date the illness began and the symptoms.
  10. Take the sick person for medical attention as soon as warning signs appear.

Effective prevention requires the cooperation of every household and the wider community. If you suspect dengue, contact the nearest healthcare facility for guidance instead of self-diagnosing or delaying medical care. This checklist supports initial monitoring but does not replace an in-person medical examination.
